The Pediatrician and the Under Toad

We are introduced to the Under Toad in John Irving's novel. The World According to Garp. A small boy at the beach is warned of the danger of being pulled out to sea by the undertow. He conjures up the image of a malevolent amphibian, lurking unseen and intent on grabbing him. The Under Toad becomes his family's symbol for all the destructive forces waiting to pounce when least expected and disrupt the peace and security of everyday life.
In our work as pediatricians, we feel the presence of the Under Toad. Some days the threat is sensed barely or not at all. At other times we can almost feel the cold breath of something ominous on the backs of our necks.
