Dr Menna makes several important points. If a partner anticipates reduction of his or her workload, plans should be worked out in advance for equitable payment, recognizing that those activities that generate the most income are not always the ones that impose the greatest stress. His scheme is a good one, although his statement that relative values of different shifts will vary from practice to practice must be emphasized. For some, a night on call involves constant activity and interrupted sleep; for others, there is minimal disruption of routine. In some practices, an additional adjustment for overhead considerations may be necessary.
