MEDICAL RECORD DOCUMENTATION OF INDIRECT NEONATAL HYPERBILIRUBINEMIA

Dr Schwartz in to do examination and talk with mother. Normal examination. Baby doing well. Formula feeding and routine newborn orders. R. Dunn, RN

December 13, 1990. Nursery Nurse

Baby taking 30 mL of formula every 4 hours. Vigorous with strong cry. One transitional stool this shift; urinated twice. Jaundice noted; total bilirubin 11.8 at 9:00 AM. Dr Schwartz notified; ordered total and direct bilirubin, hematocrit, blood smear, and reticulocyte count for 7:00 PM. R. Dunn, RN

December 13, 1990. Nursery Nurse

Total bilirubin 13.2, with direct of 0.1 at 7:00 PM. Phoned to Dr Schwartz, along with other lab results.
